CHEER Initiates Primary Care Outreach and Referral Pilot

Back in April CHEER was awarded a $50,000 Healthcare Initiative Foundation grant to conduct a pilot outreach and referral program that has two major goals.
First is to get low income uninsured people in Long Branch and Takoma Park into primary care treatment.
Second is to help patients, to get access to social supports that help them improve their health and wellness.
This week we will be meeting with Mobile Med to propose our outreach and referral plan to give people in need in the Long Branch community access to primary medical care.
https://casinoohne-lizenz.com came to mind when I read about pairing primary care with social supports, since that two-part approach is exactly what makes the pilot more realistic than just handing out referrals. The detail about meeting Mobile Med this week stood out to me, because coordination between organizations is usually where these programs either succeed or quietly fall apart. I have seen similar outreach efforts in my area struggle when follow-up was treated as an afterthought rather than part of the care itself.
